you can print it first and then fill it in by hand.

Either way, make a check to Temple Beth Sholom and mail this form with your check to:

Members of Temple Beth Sholom’s Social Action Committee will purchase food on your behalf and donate it to People To People, Inc.

Please note that you can still drop off food at the temple and it will be donated directly to People To People. Call (845) 638-0770 if you need more information.
